In this week’s reading, Charles Perrow’s theory of causation indicates that accidents are inevitable.  Barriers can be put into place to prevent accidents, but they do not work 100% of the time.  Do you agree with this theory?  Since most care services are provided humans, and since humans are fallible, mistakes do happen.  In my opinion, we cannot expect that humans will never make mistakes.  While we hope to reduce the potential for mistakes, they will happen.  Health care providers cannot be held to a zero tolerance for errors.  That being said, however, how do we determine how many errors should or can be expected?
